Project Initiation Form Technical Help Page

This page is a PDF help resource for the Project Initiation Form.

General Instructions:
The Project Initiation form requires Adobe Acrobat Reader/Standard or Professional 7 or later. "Right mouse click" (or equivalent) the link on the Project Initiation Form page and save ("Save target As" or equivalent) to your computer. Open the file you just saved to your computer and complete the form. Email the form as an attachment to:
customer.service@pf.ucsb.edu

After saving your changes to the form, please reopen the pdf file to ensure the data was saved.
