Project Initiation Form

The Project Initiation Form is a new process that Design & Construction Services is rolling out to our clients in order to streamline the project start up process. Our goal is to organize all of the necessary information required to start a project onto a single form. This form is currently in the "Testing" phase and we would appreciate your patience and comments related to improving the function of the form.

Instructions:
"Right mouse click" (or equivalent) the link Project Initiation Form below and save ("Save target As" or equivalent) to your computer. Open the file you just saved to your computer and complete the form.

After saving your changes to the form, please reopen the pdf file to ensure the data was saved.

Email the form as an attachment to: customer.service@pf.ucsb.edu
